Medical Authorization and Liability Release

Medical Authorization and Liability Release: I authorize LiveWire Athletics, LLC and its representatives to consent to medical treatment for my child when I cannot be reached to so consent. I am fully aware that any activity involving motion, height or athletic activities create the possibility of serious injury, paralysis or even death. I further agree to hold LiveWire Athletics, LLC and its staff or volunteers harmless for any injury or resulting expense(s). I release and discharge all rights and claims against LiveWire Athletics, LLC, and its parties. LiveWire Athletics, LLC strives to provide the maximum in safety procedures and guidelines, and cannot assume responsibility for any accidents, injuries or illness that may occur.

 

Parental Consent

Parental Consent: The athlete(s) listed above has my permission to participate in the LiveWire Athletics, LLC program. In consideration for the training and coaching provided by LiveWire Athletics, LLC and its staff, and or volunteers. I understand that my son/daughter must abide by the rules and regulations set forth by the coaches and staff. I understand that violations of any of these rules may result in removal from the classes and or events. We acknowledge and recognize that hazards are present in any athletic event and that injury may result. LiveWire Athletics, LLC coaches, staff and volunteers will not be liable for injury that occurs during cheerleading practice, contests, or travel to and from cheerleading activities.

 

Photo Release

Photo Release: I hereby grant consent to LiveWire Athletics, LLC to reproduce any photo images or videos taken of myself or my child for advertising or promotional purposes in perpetuity.

 

Payment Policy for Members

Policy: Monthly tuition is due on or before the 1st of the month. July’s tuition is due July 1st for example. The tuition rate will increase by $10 on the 2nd of the month. All students who are unpaid by the 8th of the month will be dropped from class automatically. You must give notice to withdraw from class enrollment by the 15th of the previous month in the form of an email. If you do not receive confirmation of termination, please call the office. All team/class students must be enrolled in Auto-Pay for monthly tuition.
